If you wish the school to administer medicine to your child whilst at school, please click above to complete and sign this form and bring it to school along with any medicine required, ensuring that your child's full name is on the medicine. The School will not be able to administer medicine to your child without a completed and signed Medicine Administration Form.
Leave of Absence from School Form If you wish to request leave during term time, please click on the above form and complete and return this form to the school office
Nursery Admissions Request Form Your child can join Fairlight Primary & Nursery School at the age of 4+, the September after their fourth birthday. If you would like to apply for a place with us please click on the above form and complete and return this to us at the school office
Breakfast Club Registration Form If you would like your child to attend our Breakfast Club, please click on the above form and complete and return it to the school office
Allergy/Intolerance/Texture Modified Diet Referral Form If you child has an allergies or food intolerance please click on the above form and return it to admin@caterlinkltd.co.uk
If your child has head lice please inform your child's class teacher, so that we can request that all parents check and treat their child/children |
