General Medication Form

Medication should be administered by the parent/carer where possible, before dropping your child and when collecting them. If there is a particular reason why painkillers need to be given in the middle of the day, please contact Juliana in the school office, before 12.30pm. If the medication is prescribed by the child’s GP, (dated/named) & needed in the middle of the day, fill in & submit the form here. Then bring the medication to the school office. If it's antibiotics that need to be taken 3xdaily, please given them at home. Many thanks.
